Forms Troubleshooting
Due to the default settings in various web browsers for the support of PDF forms, the recommended web browser to view and print PDF forms is Internet Explorer 11 or later.
Viewing Forms
If you are experiencing problems viewing forms on our website and are using Google Chrome, Mozilla Firefox, or Microsoft Edge (new with Windows 10), either use Internet Explorer as an alternative browser or follow the steps below:

Right click on the link to the form

Click Save target as… or Save link as… (depending on the browser being used)

Select a location to save the file on your computer and click Save

Locate the saved file on your computer

Right click on the file and click Open with

Click Adobe Acrobat Reader
To change the settings for how PDF files are opened in your browser, which will likely eliminate the need to save each PDF file to your computer, follow the steps below for the respective browser:
Google Chrome

Type chrome://settings/content in the address bar and press enter

Scroll down and click on Additional content settings

Click on PDF documents

Select Download PDFs
Mozilla Firefox

Click the Open Menu button in the upper-right-hand corner of the webpage

Click Options
-
Click on the Applications tab

Change the Action column for the Portable Document Format (PDF) to Use Adobe Acrobat (default)
Microsoft Edge

Right click any PDF file saved on your computer

Click Properties
-
In the General tab, click Change to the right of Opens with

Select Adobe Acrobat Reader and click OK

Click OK in the Properties dialog box
Printing Forms
If you are experiencing problems printing forms on our website and are using Google Chrome’s PDF viewer, try one of the solutions below:
- Use a different web browser, such as Internet Explorer;
- Print all pages of the form instead of selecting only certain pages; or
- Change the settings in Google Chrome so that the Google Chrome PDF viewer is not used (See Viewing Forms instructions above.).
