Forms Help

If you are experiencing problems viewing forms on our website and are using Google Chrome or Mozilla Firefox, either use Internet Explorer as an alternative browser or follow the steps below to change the settings for your respective browser:

Google Chrome
  • Type chrome://settings/content in the address bar and press enter
  • Click PDF Documents towards the bottom of the page
  • Move the slider for Open PDFs using a different application to the right
Mozilla Firefox
  • Click the Open Menu button in the upper-right-hand corner of the webpage
  • Click Options
  • Click on the Applications tab
  • Change the Action column for the Portable Document Format (PDF) to Use Adobe Acrobat (default)
