Forms Help

If you are experiencing problems viewing forms on our website and are using Google Chrome or Mozilla Firefox, either use Internet Explorer as an alternative browser or follow the steps below to change the settings for your respective browser:

Google Chrome
  • Type chrome://plugins in the address bar and press enter
  • Disable the Chrome PDF viewer
  • Enable Adobe Reader (only if listed – otherwise, this step is not necessary)
Mozilla Firefox
  • Click the Open Menu button in the upper-right-hand corner of the webpage
  • Click Options
  • Click on the Applications tab
  • Change the Action column for the Portable Document Format (PDF) to Use Adobe Acrobat (default)
